Social Services - Care Planning - Discharge Planning

At Silver Springs Care Center, our social services department works with residents, families and community agencies to facilitate a positive adjustment to the life at the facility and prepare for services at the next site of care, ideally re-entry into the community and living at home.

Upon discharge follow up phone calls are made for 90 days to ensure successful community re-entry. The Social Services department consists of three highly qualified social workers who are always available to assist families and residents in adjusting to the medical or psychological changes, whether the placement is temporary or permanent.
